For Companion Card Holders:
If you possess a Companion Card, you can obtain a key for free. Simply visit the MLAK order form for Victorian Companion Card holders and fill it out.
No Companion Card? No worries!
Even if you don’t have a Companion Card, you can still obtain a key by following these steps:
Fill out the attached form or use the online form provided here to complete the MLAK order form.
Attach one of the following documents as proof of eligibility:
- Copy of your Disability Card
- Copy of your Disability Parking Permit
- Letter from your doctor
- Letter from a disability organisation
Include a declaration of eligibility.
Pay $25.00 per key using a card during the application process.
